What are the current options for this, and how long do they take?

I was hoping to do the proctored test kits as that seems the most flexible, but we are flying through Antigua and it looks like they are not accepted there.

The BVI Health Services Authority Clinics on Virgin Gorda and Jost Van Dyke as well as the hospital in RoadTown will all do PCR tests. Turn-around time will be dependent on volume. We got all three of our tests (in April and May) back in about 24 hours. There are several other clinics on Tortola that do testing as well.

My wife and I are headed down tomorrow and are curious if anything has changed?
We are both vaccinated and will be heading back to SJU on the 16th. So I take it we will still need to get a negative PCR test prior to arriving back in SJU transferring to a flight to Miami. Are there any places with a cheaper test than others? We are on a charter boat so I guess we need to sail back to a location that offers the test and take it a few days prior to our flights.

You can simply do a rapid antigen test. If you had time, I'd suggest bringing an eMed.com test. There are lots of places on Tortola to get an antigen test or you can go for a PCR test at the hospital.

When doing a PCR test at one of the public clinics (thinking Virgin Gorda) do we have to return at a later date to collect the result, or are results emailed? If emailed, are emails generally accepted or do we then have to find a place to print them? If we do need to print, does anyone know where we could do that?

Emailed. It's better to have them printed but it's only shown at the airport/ferry dock so you can get by without the paper. Usually the charter office or hotel office will print things for you.

If you're taking the ferry back to St. Thomas, you need to upload your test results to the USVI portal. They'll ask to see your green QR code at the ferry dock and then again when you arrive in St. Thomas.
