EHAS Form Question

On the EHAS form we need to upload both the vaccine and booster information separately. Our vaccine record card has all three shots on one. Will this be acceptable on the form as they ask for the vaccines and booster proofs separately??

Yes, just upload the same image in both places.

That's the way mine is too. I'll just load same PDF if still needed this coming May.
