After I submitted my application to ehas and paid for the insurance, I realized I stupidly uploaded the wrong download for my vax card. So I know that form will be denied. Since I already paid the $15 insurance fee, how do I re-submit the form with the correct info without having to pay another $15?

It indicates on the website that they will not process the cc payment until you are approved. Check your cc. If You don’t see a charge then I would think you are ok to just start over.

They will not double charge you if you resubmit. Others have had to resubmit and were not double charged.
