After quite a hiatus from SXM, we are planning a trip for Late May. We are a doing a vacation package through AA. They do not offer travel insurance on packages, just pre-departure insurance?? Any suggestions for travel insurance including emergency medical transport back to the US.

Emergency medical transport is a big issue. We ALWAYS buy it these days. You can check with TTOL sponsor Insure my Trip, though the link here on TTOL, on the sponsors page. They sell all kinds of travel insurance. If you book through the link here on TTOL, versus going directly to their site, the price to you is the same, but TTOL gets a small commission.

As another option for you, we use GeoBlue for medical coverage when we are out of the country. Also provides medical evacuation transport back to the US when necessary. Both hospitals on the island as well as many doctors and other providers are in their network. While we have never had to use it, our understanding is that as long as the provider is in the GeoBlue network you just show them your card and no copay.
