Whale Tail--I think Air Transat is incorrect, as Cdn Couple stated. Which is part of the problem, the SXM government does not give clear direction of what is required, but pretty sure that the form is NOT required, unless something has changed.
Carol - The screenshot that I provided says "Official Government of Sint Maarten"
The screenshots provided by CdnCouple are from a website called "vacationstmaarten.com" and I don't believe that site is an official government website.
