I decided to send an email to see if I could get clarification The resoponse came from vacationstmaarten.com, which makes me question if it is 100% accurate. The form and vacationstmaarten.com seem to be intertwined. If you will remember, part of the reason for the form was to guage travel interest and decisions. I do remember Grisha posting (I think on facebook) that it was not required.

Thank you for contacting vacation sxm, although the Ed form isn't mandatory at the moment it is still best to have it done before you arrive to St Maarten it saves you time from standing and waiting in line because you will still need to have it completed when you arrive. Hoping that I've informed you adequately.
