I am asking for some assistance for my October 31 trip. I know the requirements but am unable to upload the form required by Sxm. I realize that I can't fill out now and need my test but I cannot get the form all I get is read the instructions and when I do and i try to get the form it sends me back to read instructions.
I sent an email last night to Sxm and did not get a response. Any help or contact information is greatly appreciated.

I just went to stmaartenehas.com, and where it said "Apply Here", there is a note to read instructions, but the form is below that. They don't want you to fill it out until after you have the results from your test.

Phillygirl7, I might have it wrong, but I think what he is saying is that every time he clicks on the link "fill in the application form" it takes him right back to "read the travel requirements". He understands that he doesn't need to complete it until he has his test results and just prior to his flight. I believe he was just checking his access to everything and found that the site wouldn't allow him to access the form. Maybe when he responds than we will know more.

I am not sure it is anything you can download. You input the information and when done, it submits to the Health Department. I gave a different website than what he was looking at, that may be the issue. I use the stmaartenehas.com website and have no problems getting to the form, and can start keying in data, but it won't save it unless you finish it with all the test information included, your passport info, where you are staying at and for how long, have you been anywhere else for the preceding 15 days, etc.

Perhaps he didn't scrolll down the page, as when I went to it, I got there, and it still said 'read the travel requirements', but you scroll down that same page and there was the form to fill in.

I did scroll down to the bottom and never saw the form. I repeated several times and got the same result. I tried again about an hour later and got the same result. I sent an two emails to Sxm one wednesday and the second Thursday and they did not reply shows how much they care.
I was really determined to make the trip my wife not so as several people we know who we're going the same time have cancelled. The virus insurance was the last straw I am cancelling our 2 week trip. I do not think the powers to be care about tourism and it will hurt the hard working people of Sxm.
This will be the second year we missed in the last 16 years the only other was due to Irma.

I do have a couple of forms I have to access (through work) that google chrome will not work on. I have to be on Firefox or Internet Explorer. Maybe try from google chrome and see if that makes a difference. The virus insurance is not what bothered me -- way harder and onerous to get the Covid test. It will take most of my day going one state away for that test. I really feel for the island and its businesses and residents and will do what it takes to get there. 3 days was not feasible, 5 days makes it much more workable to get the test results.
