Your CDC vaccine card should state which vaccine you received along with the dates. I'm sure they'll want a scan of that card to issue the approval.

But you said you'd have to "say" what vaccine you got. I seriously doubt that they would be willing to take your word for it.

By "saying " would be on the question on EHAS and your card submission would prove it.
