But when I check airfare (not so much the cost) but to get there from the northern east coast is 2 days traveking. Unless someone has a better idea of airfare? Our problem too - don't want to spend an entire day getting there! But direct flights don't leave many options. One flight and a short boat ride would be ok!

I second Grenada, easy to get to from the US meets most of your requirements, except maybe the shopping, no designer outlets or any of that. Kim and I have been out till the early hours of the morning, the only tourists drinking in a local rum shop and then walk 1/2 mile home along the main road without any concerns. You can also take the ferry to Carriacou for a couple days, it's a beautiful place to go if you just want to chill on the beach, not much else to do there
Drink all day at home, your friends worry about you; do it on vacation and they say "what a good time you're having". Save your friends needless worry, travel more!

I would agree with all those who suggested Grenada - it's a beautiful island with lovely people and seemingly, very safe. And while I don't know what kind of activity you might be looking for, there is a lot to do there but it's more in the hiking, exploring, discovering arena as opposed to the partying, drinking, gambling scene, though I know there is a lot of at least two of the latter options, too.
