Took the time to look into filling out the form on a test run. Just a few pointers to make it easier for you. Avoid using the auto fill as it'll stop you from advancing to the next field to fill out. It's day-month-year for everything just like your passport. I had a hard time finding where I put the Vax card picture on my drive so I converted it into a PDF and made it easier. It asks if your boosted and if you check yes then they want to see proof again even if the booster is shown on first card you attached. Now I'm all set to when I have to do it next week.
