We have tried the on-line forms, we have tried calling, we have tried sending emails to the president, as suggested here, but to date we have received nothing. Not only that, the credit expires one year from the date of purchase, which was in early January 2017. Once we heard that SXM was cancelled, I went ahead and scheduled rotator cuff surgery in November.
